경추체 제거술 후의 수술결과와 위관절증에 대한 흡연의 영향 The effects of smoking on perioperative outcomes and pseudarthrosis following anterior cervical corpectomy

Brief review

JNS에서 10월호에 출판된 재밌는 논문 입니다.
결론부터 말하자면, 경추체 제거술 후의 수술결과와 합병증(pseudarthrosis)에 흡연여부가 유의하게 영햐을 미친다는 내용 입니다.

흡연여부에 따라 피험자를 비흡연자/ 끊은지 최소 1년이상된 자/ 흡연자  의 3그룹으로 나누었고, 이 피험자들의 30일이내 합병증 발병, 출혈량, 재원일수 등의 항목을 조사하였습니다.

결과적으로 흡연여부는 더 높은 합병증 발병률, 더 긴 재원일수에 영향을 미치는 risk factor로 밝혀졌으며, 감염 및 pseudarthrosis등의 합병증 발병에 특히 영향을 미친다고 합니다.
흡연의 해로움을 객관적 데이터로 보여준 논문입니다.

Object
Smoking is one of the leading causes of preventable morbidity and death in the US and has been associated with perioperative complications.
-To examined the effects of smoking on perioperative outcomes and pseudarthrosis rates following anterior cervical corpectomy.


Methods
Subjects: adult patients underwent anterior cervical corpectomy (2006~2011)
Grouping:
1) nonsmokers
2) quitters (quit for at least 1yr)
3) current smokers

Statistical analysis: using multivariate analysis
(To define the relationship between smoking and blood loss, 30-day complications, length of hospital stay, and pseudarthrosis)



















Results

Total 160 patients were included in the study.
(nonsmokers 49.4%, quitters 25.6%, current smokers 25.0%)
30-day complication rate: 20.0% (pseudarthrosis: 7.6%)
Mean blood loss: 368.3ml
Mean length of stay: 6.5days.

-Current smoking status was significantly associated with higher complication rates (p < 0.001) and longer lengths of stay (p < 0.001).
-Current smoking status remained an independent risk factor for both outcomes.
                                     
The complications that were experienced in current smokers were mostly infections (76.5%).
(Significantly greater than in nonsmokers and quitters (p = 0.013))
-Current smoking status was also an independent risk factor for pseudarthrosis at 1-year follow-up (p = 0.012).































Conclusions

Smoking is independently associated with higher perioperative complications (especially infectious complications), longer lengths of stay, and higher rates of pseudarthrosis in patients undergoing anterior cervical corpectomy.

특히 "최소침습적인 척추수술은 전통적인 수술법보다 비용면에서 효율적인가?"라는 제목의 논문이 눈에 띄네요.

내시경을 이용한 경피적 요추 절제술에서의 학습곡선 Learning curve for percutaneous endoscopic lumbar discectomy

Brief review

이 논문은 PELD에서의 learning curve에 대한 논문 입니다.
요새 계속 PELD learning curve에 대한 논문을 보고 있는데, 각 논문별로 언급된 항목에 조금씩 차이가 있네요. 여기서는 수술시간, 1년 후의 임상적 결과, 수술 성공률, 수술 실패율, 합병증 발생률, 재발률 등의 항목을 조사하였습니다. 
이 논문이 2008년에 출판되었는데, 최근에 출판된 PELD learning curve관련 논문들과 큰 차이는 없어보입니다.

Clinical success를 아래의 기준으로 판단했습니다.
1. 통증지수(VAS)의 향상률이 2점 이상
2. 장애지수(ODI)의 향상률이 25% 이상
3. 환자의 주관적인 회복률이 50% 이상
4. 주요한 합병증이 없음
그런데 주관적인 회복률을 어떻게 평가했는지 안나와있네요.. clinical outcome 평가 시 처럼 환자에게 전화하여 환자가 느끼는 회복률을 적용한 것 일까요

그리고 특이한점은 일반적으로 의학통계에 널리 사용되는 SPSS가 아니라 SAS라는 프로그램을 사용하여 분석을 했습니다.

Background of data
Percutaneous endoscopic lumbar discectomy (PELD) is one of the surgical options for soft lumbar disk herniation, but the learning curve is perceived to be steep.


Methods















Subjects: 51 PELD cases performed for single-level intracanalicular lumbar disk herniation causing radiculopathy.
Divided into 3 groups: each 17 patients
Evaluating parameters: operating time, failure rate, complication rate, and 1-year reherniation rate.
1 year clinical success rate was assessed by telephone interviews






























Results
4 patients required subsequent open discectomy due to PELD failure.
2 minor complications.
1 year clinical success: achieved in 42 of the 47 patients
Reherniation developed in 5 patients
-A significant reduction in operating time was observed after 17 patients had been treated (p = 0.0004).
Failure rate, complication rate: no significant differences between the 3 groups
1year clinical success rate, reherniation rate: no significant differences



Conclusion

The PELD learning curve seems to be stable and acceptable with proper pre-PELD training.

합병증은 최소침습 척추수술의 초기 학습곡선과 연관성이 있다 Complications associated with the initial learning curve of minimally invasive spine surgery: a systematic review

Brief review

최소침습적인 척추수술의 Learning curve(학습곡선)을 주요 항목으로 하여 체계적 문헌고찰을 시행한 연구 입니다.
Pubmed에서 검색된 논문을 정해진 프로토콜대로 리뷰하여 최종적으로 15개의 논문이 선택되었습니다.
각 연구별로 complication, procedure time, fusion rate, estimated blood loss, level of evidence, sample size, intervention, study design등을 조사하여 정리하였습니다.
decompressive procedure에서는 durotomy, 그리고 fusion에서는 implant malposition,neural injury, and nonunion가 가장 흔한 합병증이라는 결과가 나왔습니다.
또 수술 후의 전체적인 합병증 발병률은 11%입니다.

BACKGROUND
-There is an inherently difficult learning curve associated with minimally invasive surgical (MIS) approaches to spinal decompression and fusion.
-The association between complication rate and the learning curve remains unclear.


QUESTIONS/PURPOSES
-Evaluated the learning curves of MIS procedures for the spine
-Defined as the change in frequency of complications and length of surgical time as case number increased, for five types of MIS for the spine.


METHODS
Searching protocol: PubMed database
-terms: "minimally invasive spine surgery AND complications AND learning curve”

Clinical outcome and learning curve metrics were categorized for analysis by surgical procedure (MIS lumbar decompression procedures, MIS transforaminal lumbar interbody fusion, percutaneous pedicle screw insertion, laparoscopic anterior lumbar interbody fusion, and MIS cervical procedures).

Procedure time and complication rate: the most consistent parameters to evaluate the learning curve

total 15 original studies are remained.

 

RESULTS
-The most common learning curve complication for decompressive procedures was durotomy.
-For fusion procedures, the most common complications were implant malposition, neural injury, and nonunion.
-The overall postoperative complication rate was 11% (109 of 966 cases).
-The learning curve was overcome for operative time and complications as a function of case numbers in 20 to 30 consecutive cases for most techniques discussed within this review.


CONCLUSIONS
-The quantitative assessment of the procedural learning curve for MIS techniques for the spine remains challenging because the MIS techniques have different learning curves and because they have not been assessed in a consistent manner across studies.

-Complication rates may be underestimated by the studies we identified because surgeons tend to select patients carefully during the early learning curve period.


-The field of MIS would benefit from a standardization of study design and collected parameters in future learning curve investigations.

PURPOSE
To evaluate the differences of learning curve for PELD depending on the surgeon' s training level of minimally invasive spine surgery.


METHODS
Reviewed the medical records: 120 patients with sciatica and L4-5 disk herniation who underwent PELD

Divide the group as surgeons’s training level
-Group A: surgeon with little professional training of PELD
-Group B: surgeon with 2 years of demonstration teaching of PELD

Patients can be divided into 3 stages: 1–20(earlier stage) / 21–40(middle stage) /41–60(latest stage)


RESULTS
Age, Sex, symptom duration, herniated disc location, rates of smokers/ patients with accompanying disease/ history of injury: No statistically significant differences




















Op time: Significant differences (p=0.000)
-significant differences were observed between the 1-20 patients group and 41-60 patients group in Group B (p=0.041), but there were no significant differences among the 1-20 patients group, 21-40 patients group and 41-60 patients group in Group A


Hospital stay: Significant differences (p=0.026)
-Significant differences in the 1-20 patients group between Group A and Group B (p=0.011)










Reoperation rate: Significant differences (p=0.050)
-Significant difference was observed between the 1-20 patients group and 41-60 patients group in Group A (p=0.028) but there were no significant differences among the 1-20 patients group, 21-40 patients group and 41-60 patients group in Group B.












Clinical outcomes: Significant differences between pre/post VAS back, leg, JOA score. 
-Higher improvement in the VAS leg score was observed in Group B than Group A (p=0.031).

















CONCLUSIONS
-The surgeons' training level of minimally invasive spine surgery was an important factor for the success of PELD

-Especially the demonstration teaching of PELD for the new minimally invasive spine surgeons.

우리들병원 이준호 진료부장, "요통, 그 종합적인 해석"

요통, 그 종합적인 해석

요통은 감기나 두통처럼 일반 성인들이 겪는 가장 흔한 질병 중 하나이다. 3명 중 2명이 한 번쯤은 이 요통으로 결근을 하거나 병원을 찾는 경험이 있다고 한다.

중년 성인의 경우 병원에 내원하면 의료진과의 문진 후 일반 방사선 촬영을 하고요추 * *번 사이 추간판이 좁아져서란 말과 함께

“디스크 끼가 있다는 말을 듣고 MRI 등 정밀 검사를 권유받는 경우가 있을 것이다.

흔히 일반인이 언급하는디스크란 의학 용어로추간판 또는 수핵 탈출증을 일컫는다. 이는 탈출된 수핵에 의해 하지로 내려가는 신경근 부위가 압박받아 다리 저림 증상을 유발, 경우에 따라서는 발목 또는 발가락 등의 부분 마비 증상까지 유발해 실제 수술을 요하는 상태를 말한다. 전체 요통 환자의 10% 내외의 드문 케이스다.

따라서 요통을 모두 디스크라고 볼 수 없지만 자칫 일반인들 사이에 잘못 알려져요통증상이디스크라는 질병으로 변질돼 용어 자체에 혼동을 가져오고 지나치게 요용되는 경향이 있다.

요통의 원인과 노화에 따른 요추부의 구조적 및 기능적인 변화는 다양하다. 단순히 허리를 무리하게 잘못 사용해 근육 경련이나 관절 강직을 일으켜 나타나는 소위요추 염좌’, 추간판 및 척추 관절의 심한 퇴행으로 인해 해당 척추 분절의 불안정 및 비정상적인 운동이 야기된 척추 불안정증까지요통의 범주는 매우 역동적이다.

따라서 그 치료 선택에 있어서 질환 자체도 중요하지만 자신의 증상 정도, 일상생활의 제한 정도, 전신 컨디션 등을 복합적으로 고려해 의료진과 논의 후 적절한 선택을 택해야 할 것이다. 제반 검사 후 방사선학적으로는 허리병이 심하다고 들었는데 내가 증상을 별로 못 느끼고 생활에 제약이 없다면 적절한 휴식과 물리치료 등의 보존적인 치료를 우선 해 볼 수 있다.

반대로 병원에서 허리 디스크 때문에 신경이 눌리지는 않았다고 들었는데 그 통증이 빈도도 잦아지고 정도도 갈수록 심해진다면 수술적 요법을 포한 적극적인 치료를 고려해야 할 것이다. 치료 선택에 유연성이 있기 위해서는 의료진의 권유도 중요하지만 반드시 환자가 주체가 되어야 한다. 단순히 옳지 못한디스크끼란 말에 의해 자신이 겪는 증상의 본질을 제쳐 놓고 혼동된 질병 명에 치료에 자신을 억지로 맞추면 안 된다.

검게 변한 내 디스크만성 요통의 근원인가?

긴 연휴가 끝나고 나니 연휴 내내 고생한 중년 주부들의 내원이 잦은 편이다. 평소에도 일 년에 한 두 번씩 허리가 안 좋아서 고생한 적이 있는데 이번 연휴 동안 무리했더니 다시 도졌다는 것이다. 아무 검사도 안 해봤냐고 물으니 전에 근처 병원에서 x-ray 한 번 찍어 봤는데 의료진이 사진 상으로는 별 문제점이 발견되지 않았다고 해 침만 맞아 왔다고 한다. 평소에 활동할 때 허리만 많이 아프고 다리 아픈 것은 별로 못 느낀다고 한다.

이런 환자들에게 MRI 등의 정밀 검사를 시행해 보면 허리 한 두 마디에 소위 검은 디스크 (black disc)의 소견을 보이는 경우가 꽤 있다. 이른바 추간판 내장증 (degenerative disc disease)이라고 불리는 이 소견은 하지로의 방사통 없이 요통을 주로 일으키는 상태를 말한다. MRI 소견상 신경근 압박 등의 소견 없이 추간판 높이도 정상인 못지않게 꽤 유지되는 경우도 있어 그 정도를 단순한 x-ray 등으로 판단하기는 어렵다.

이렇게 추간판 내장증, 소위 검은 디스크라는 병에 대해선 아직 구체적인 진단 기준, 병태 생리, 자연 경과 등이 추간판 탈출증 같이 밝혀진 바가 없어 환자들에게 올바르게 설명하고, 치료 방법을 제시하는 데에 어려움이 그나마 학계에서 어느 정도 동의 받는 것은 수핵의 퇴행으로 야기된 각종 염증 또는 통증 유발 화합물질 들이 통증 수용체가 분포하는 섬유륜의 바깥쪽에 균열 (fissure, high-intensity zone on MRI) 을 통해 침범하여 요통을 유발한다는 가설이다.

하지만 이러한 가설을 근거로 섬유륜에 분포하는 통증 수용체를 없애려는 목적 하에 시행해 본 섬유륜 성형술 (annuloplasty) 또는 고주파열 치료술 (IDET), 신경성형술 (epidural neurolysis) 등의 효과가 만족스럽지 못한 경우가 많아 이러한 가설만이 병적 상태를 해석하는 데 전부가 아닐 것이라는 게 지배적이다.

만일 내 디스크가 검다는 설명을 듣고 소위 철심으로 고정술을 받을 것을 권유 받았다면 재고 해볼 필요가 있다. 추간판이 검게 변하는 것은 대개 30세 이후에는 남녀 구분 할 것 없이 허리 한 마디쯤에 다 나타날 수 있는, 어쩌면 나이를 먹는 것과 비슷한 현상으로 생각할 수 있다. 여하튼 과학적인 연구 조사 결과를 토대로 확실한 치료 방법이 정해지지 않은 상태를 두고 무분별하게 수술을 위한 침습적인 치료가 선행되어서는 안 된다.

일단 위 양,음성 반응의 가능성은 있지만 추간판 조영술 (discography) 등으로 요통과의 연계성을 어느 정도 확정하고 그 증상 정도가 본인 판단에 인내할 수 있는 정도라면 척추 근력 강화 운동 등의 물리치료부터 장기적으로 꾸준히 선행해 볼 필요가 있다.

만일 그 정도가 도저히 참을 수 없는 정도라면 좀 더 침습적인 치료를 요할 수도 있는 데 경우에 따라서는 골 유합술 까지 요하는 경우도 있으나 병 자체가 확고한 과학적 근거가 부족한 관계로 이에 대한 수술적 치료 역시 실험적일 수 있음을 주지해야 할 것이다.

아무리 검게 변해버린나쁜디스크지만 결과적으로 남의 것을 쓰는 것보다는 나쁜 내 것을 놔두는 것이 더 나을 수도 있기 때문이다.

우리들병원 안용 병원장 spine논문 게재 Radiation exposure to the surgeon during percutaneous endoscopic lumbar discectomy: a prospective study

우리들 병원 안용 원장님의 SPINE저널에 게재된 논문 입니다.
내시경 수술을 시행하는 의사와 스텝들에게 방사선 피폭은 매우 민감한 사항 입니다. 논문에서는 경피적 내시경 요추 절제술을 시행하는 동안의 방사선 피폭량을 분석하고 일년에 허용 가능한 수술 건수를 산정했습니다.

STUDY DESIGN
A prospective study.


OBJECTIVE
-To determine the radiation dose to which the surgeons are exposed during percutaneous endoscopic lumbar discectomy (PELD)
-To calculate the allowable number of cases per year.


SUMMARY OF BACKGROUND DATA
-Minimal invasiveness can be achieved through the use of fluoroscopy and endoscopy.
-The radiation dose to the surgeon during PELD is unknown.


METHODS
-The occupational radiation dose absorbed by 3 spinal surgeons performing 30 consecutive PELDs (33 levels) during a 3-month period was evaluated.
-Transforaminal PELDs were performed according to the standard technique.
-Part of measured the radiation exposure: neck, chest, arm, and both hands of the surgeons
-Occupational exposure guidelines of National Council on Radiation Protection & Measurements were used to calculate the allowable number of procedures per year.


RESULTS
-mean operation time: 49.8 minutes / mean fluoroscopy time was 2.5 minutes.
(No significant correlations)

Calculated radiation doses per operated level
-neck: 0.0785 mSv /chest: 0.1718 mSv /right upper arm: 0.0461 mSv/ left ring finger: 0.7318 mSv/ right ring finger: 0.6694 mSv.

allowable number of cases per year
-using a lead apron: 5379 operations can be performed per year
-without using a lead apron: 291 operations can be performed


CONCLUSION
-Without radiation shielding, a surgeon performing 291 PELDs annually would be exposed to the maximum allowable radiation dose.
-Given the measurable lifetime radiation hazards to the surgeon, the use of adequate protective equipment is essential to reducing exposure during PELD.
