Facet orientation and tropism: associations with facet joint osteoarthritis and degeneratives

Brief review

후관절의 방향과 비대칭성, 골관절염 그리고 퇴행성 척추전방전위증 간의 연관성을 평가한 연구로, 대동맥의 석회화를 평가하기 위해 3529명의 환자를 분석했던 Framingham Heart Study의 부수적인 연구라고 합니다.
요통이 있는 188명 환자들의 CT사진을 평가했고, 특이한점으로는 후관절의 관절염을 4개의 grade(normal, mild, moderate, severe)로 나누어 평가했습니다. (figure 2 참고)
, 후관절 방향, 비대칭성, 관절염 및 퇴행성 척추전방전위증 간의 연관성을 평가하기 위해 다중 로지스틱 회귀분석 모델을 사용했습니다.
결론적으로, 후관절의 방향과 관절염, 퇴행성 척추전방전위증 간에는 통계적으로 유의한 연관이 있었지만 후관절의 비대칭성은 관절염 및 퇴행성 척추전방전위증과는 관련이 없는 것으로 밝혀졌습니다.

+cross-sectional study(횡단면적 연구): 연구대상에 대해 2회 이상의 추적조사를 시행하는 longitudinal study(종단연구)와는 대조적으로, 특정시점에 한번만 자료수집을 하여 결론을 도출하는 연구입니다 (specific point in time!).
다른 연구방법에 비해 비교적 진행하기 쉬운 형태이므로 주로 대규모 연구를 시작하기 전에 예비연구, 조사의 개념으로 시행한다고 하네요.

STUDY DESIGN
Cross-sectional study.


OBJECTIVE
To evaluate the association between lumbar spine facet joint orientation, facet joint tropism, and facet joint osteoarthritis (OA) and degenerative spondylolisthesis (DS) identified by multidetector computed tomography in the community-based Framingham Heart Study.


SUMMARY OF BACKGROUND DATA
The association between lumbar facet joint OA, DS, and facet orientation and tropism remains unclear.
This study was an ancillary project to the Framingham Heart Study(to assess aortic calcification).


METHODS
Subject numbers: 188 patients
-To assess radiographic features associated with low back pain.

Evaluation method
-Facet joint OA: using a 4-grade scale (L3-L4, L4-L5, and L5-S1 levels).






























-The association between facet joint OA, DS, and facet orientation, and tropism: using multiple logistic regression models adjusting for age, sex, and body mass index.


RESULTS
At each spinal level the facet joints with OA were more sagittally oriented than those without OA, but the difference was statistically significant only at L4-L5 spinal level (P = 0.0007).

Facet tropism did not show an association with facet joint OA at any spinal level.
Facet orientation was significantly associated with DS (0.89 [0.84-0.94]).
Facet tropism showed no association with DS.


CONCLUSIONS
The current study confirms a significant association between sagittal orientation and OA (of the lumbar facet joints at L4-L5) and DS.

Facet tropism was not associated with occurrence of facet joint OA or DS
